Office Protocol


The physicians of Forest Lane Pediatrics would like to welcome everyone to our pediatric offices. We hope the following information is helpful.
Our practice is growing and we would like to help our patients and parents make a smooth transition in regard to changes that will be taking place. We hope the following information is helpful in informing everyone that comes to our office and provides a more pleasant environment.





Immunization Policy

At Forest Lane Pediatrics, we believe that all children should be fully immunized unless there are medical contraindications. Therefore, we are no longer accepting new patients/families unless they are willing to fully comply with the recommended timetable for vaccine administration per the American Academy of Pediatrics. The only exception would be:  Hepatitis B administration could be initiated at no later than 10 years of age and completed by age 12.  We believe this policy advocates for children and their best interest. 

Automated Phone System

Our office uses an automated phone system to help direct telephone traffic. Please note the prompts as you listen to the system. Phones will be turned on at

  1. Phone messages are prioritized according to urgency.
  2. Appointments requiring an exam that day will be answered within a 1 hour time frame.
  3. Non-urgent calls will be answered within 2-3 hours.
  4. Lengthy calls, such as: feeding, sleeping, or behavioral questions will be answered by the end of the day.
  5. The prescription request or refill line will be answered hourly.
  6. The referral line will be checked daily – you are responsible to know if you need a referral to see a specialist. Please call the referral line 72 hours in advance of the needed referral.
  7. Medical record requests left on the phone system will be answered daily. Please allow 72 hours to have records available and these can be picked up, faxed or mailed. e will not send out any records without a signed records release form.
  8. You will still be able to get an appointment in the same manner as in the past. lease note we now have two physicians in the office. If one of the physicians is busy and your child is sick and needs an urgent appointment, he or she may be required to see a different doctor. All well exams may be scheduled with your regular physician.

Email Policy

We will accept email questions to be answered the next regular business day. We will ask you to fill out an email policy form that states what will be addressed via email response in order to comply with HIPPA regulations.  Please e-mail us via this website under "Contact Us." 


    After Hour Calls

    All after hour calls will be answered in a timely manner by the physician on call. There will now be a $25.00 physician call charge for any call after

    Late Appointment Policy

    If you are more than 30 minutes late for your appointment, you may be asked to reschedule for a later date. If your child is sick, you may wait in the office and be worked in between patients. Please note there will be an extended wait time if you are late for your appointment.

    Same Day Appointments

    If you call the office, and need a sick appointment on the same day, there will be a wait time as we will be working you in between the regular scheduled appointments.

    Forms

    Various forms and letters are often lengthy and may take extra time to be filled out. Please allow 48 hours for forms or letters to be completed by our office. There will now be a $15.00 charge for letters or forms needing more than a signature.

    Medical Records

    There will be a $25.00 charge for copying of your child’s chart. If you want records sent to another physician, then there will be no charge in that instance.

    Appointment No Show Policy

    Failure to cancel your appointment within 24 hours notice will result in a $25.00 charge.  This charge must be paid prior to scheduling your next appointment.

    The office protocol will be updated periodically as the practice grows and changes will be made accordingly.
